- This series contains forms submitted to the State Board of Charities by poorhouse officials from most counties in the state. Forms provide information regarding children transferred from poorhouses to orphanages, apprenticeships, or families willing to act as guardians. "General Inquiry" forms provide aggregate statistics for each institution. "Personal Inquiry" forms record data for each child placed outside an institution: name; sex; age; if native or foreign born; if orphan; physical health, occupation, and status in current living situation.

Files prior to the 1920s consist almost exclusively of inspection reports of private institutional homes for children prepared by the State Board of Charities. After the late 1920s, the files contain increasing amounts of administrative correspondence and less detailed reviews of physical facilities, client care, and management and operations.
