- This fragmentary series consists of several types of medical records. A register of operations lists detailed information regarding surgical procedures performed at the facility. This register notes that the surgical facility was closed January 1980 and contains a list of nurses and anesthetists who worked in the facility along with their service dates. Series also includes an x-ray logbook consisting of compiled reports of results of tuberculosis tests; two small menstruation logbooks; and two small pharmaceutical logbooks.

- This series was created by the medical inspector for the State Hospital Commission (later the Department of Mental Hygiene) to document observations made on inspection visits to state hospitals, state schools, hospital psychiatric wards, and private mental health care institutions. Narrative notes include date and time of arrival; professional staff on duty; list of staff vacancies; patient census statistics; notes on new admissions; discharges; deaths; and special interviews with patients about complaints regarding care and treatment, accidents, and injuries.
