- This fragmentary series consists of several types of medical records. A register of operations lists detailed information regarding surgical procedures performed at the facility. This register notes that the surgical facility was closed January 1980 and contains a list of nurses and anesthetists who worked in the facility along with their service dates. This fragmentary series consists of several types of medical records. Register of operations lists date of operation, register number, major or minor, name of patient, room number, name of surgeon, name of assistant, age of patient, anesthetist, anesthesia, name of surgical nurse, postoperative diagnosis, circulating nurse, operation type, drainage, anesthesia unit, surgical code number, complications, hospital number, specimen to lab, and postoperative condition. Register notes that the surgery facility was closed January 1980 and contains a list of nurses and anesthetists who worked in the surgery facility with their service dates. Series also includes an x-ray logbook consisting of compiled reports of results of tuberculosis tests, two small menstruation logbooks, and two small pharmaceutical logbooks.
