Summary
- The dispensary record, tracking the health and medical treatment of inmates, was a source the physician's monthly and annual reports. Included are: inmate number; division; inmate name; admission date; diagnosis/physical condition; treatment; name of doctor (rarely); occasional remarks (such as the inmate is to see the doctor again or to see another doctor); and discharge date or other disposition (e.g. to isolation ward, to hospital).

Summary
- The series consists of brief memorandums from physicians to prison authorities usually recommending appropriate medical care or end of need for medical care for incarcerated patients that the physicians have examined. Many memorandums recommend that an incarcerated individual be mustered out of the military squad because of a medical problem. Memorandums include date, addressee, consecutive number, last name of the incarcerated individual, and physicians name and title. The name of physician is often omitted.
