In one questionnaire, the superintendent requests inmates to discuss what they like about the institution; in the other, what they do not like. Both questionnaires provide inmate's name, division, grade, and shop. Opinions are given on institution shops; schools; military, yards; sleeping halls; dining rooms and food; bath room; guard house; and chapel service. Records are restricted.

This series consists of inspection reports, correspondence, and related material documenting the monitoring and oversight of foster care agencies, including institutional homes for children and orphanages. Files prior to the 1920s consist almost exclusively of inspection reports of private institutional homes for children prepared by the State Board of Charities. After the late 1920s, the files contain increasing amounts of administrative correspondence and less detailed reviews of physical facilities, client care, and management and operations.

Examination form provides date; county in which facility is located; examination number(s); and answers to 26 questions on child identity, personal/family history, siblings, economic circumstances of parents, moral character, mental health, causes of dependency, and the likelihood of future independence. Often a section for remarks provides additional information.
