Summary
- The cards in this series serve as an index to patient admission and discharge registers. Data on the cards includes name, address, next of kin, facility, case number (the facility assigns a patient identification number to the patient), and state identification number (assigned by the Office of Mental Health and is one number per individual regardless of where the individual is receiving treatment). Both the Office of Mental Health (OMH) and individual facilities maintain sets of these index cards. The OMH set of cards includes master numbers for patients admitted to private psychiatric facilities through circa 1980 and the patient numbers for all facilities at which a client received treatment.

- This series consists of logbooks recording information related to x-rays taken of patients and employees of Utica State Hospital (later Utica Psychiatric Center). Each entry typically records date, number, name, and area of the body x-rayed. Some volumes contain only lists of patients to be x-rayed and the dates of scheduled appointments. The volumes may have been created to track the status of patients who had received x-ray treatment, possibly for followup.

- This series documents the activities of the Berkshire Industrial Farm infirmary. These logs were kept by nurses and include date and arrival time of patient, patient's last name, ailment and treatment, and notes regarding doctor's call or visit to the infirmary. Some registers include lists of patients who received vaccines, anti-toxins, and/or were sent to House of Mercy clinic or the hospital for examinations, operations, or x-rays (1928-1930).

- This fragmentary series consists of several types of medical records. A register of operations lists detailed information regarding surgical procedures performed at the facility. This register notes that the surgical facility was closed January 1980 and contains a list of nurses and anesthetists who worked in the facility along with their service dates. Series also includes an x-ray logbook consisting of compiled reports of results of tuberculosis tests; two small menstruation logbooks; and two small pharmaceutical logbooks.

- This series consists of reports documenting formal inspections and official visits by Department of Mental Hygiene personnel to Willard Asylum for the Insane (later Willard State Hospital). Included are meeting minutes; observation notes on buildings and grounds tours as well as visitations with staff and patients; recommendations; and requests for changes, improvements, and repairs. Some reports give detailed individual reports for each building. Most recent reports contain printed checklists and fewer narratives. Records are restricted.
