Many files contain a full-face photograph of the inmate as well as biographical information similar to the intake registers. The forms also include observational notes on the inmate's current behavior, prognosis and possible actions to be taken. Some files give more detail on religious beliefs. Records are restricted.

- These files consist of physicians' orders for individual patients to undergo protective restraint or seclusion, and of regular statistical reports detailing the use of the practice at Willard Psychiatric Center (known earlier as Willard State Hospital). The Department of Mental Hygiene and later the Office of Mental Health used these figures to provide each facility director with a monthly record showing where his or her institution stood with respect to the average for other institutions in the department.

This series contains records detailing the department's work in the screenings, in addition to correspondence and memoranda between staff relating to the department's efforts to prevent its hospital personnel from being drafted by classifying them as essential to operations.

These cards were originally part of the Inmate Identification Files. The identification envelopes for these cards are do not exist. The case files for the inmates described by the cards do exist though. Data on the card includes name, residence, number, aliases, sentencing and commitment information, crime, birth, height, weight, build, nativity, occupation, and fingerprints.
