Information is handwritten in columns on preprinted pages which were later bound into volumes for each year. Monthly reports from each institution provide the following information: name of incarcerated individual; county where convicted; crime; court; judge; date of sentence; date received at prison; term; and remarks (such as previous prison terms).

- This series consists of a daily count of the population of incarcerated individuals at the Elmira Reformatory taken Monday through Friday at various intervals. Locations at which population counts were conducted include work areas (construction, laundry, kitchen, etc.); educational classes; guard house; hospital; library; cellblocks; dining rooms; cage floor; center gate and dormitories. Also counted were the number of incarcerated individuals received or paroled. The total population of incarcerated individuals for each count is included along with the name of staff member taking the count.

- This series consists of correspondence and forms compiled by Protestant Chaplains at other prisons while conducting interviews to determine religious affiliation for inmates who ultimately resided at Dannemora State Hospital. Many files contain a full-face photograph of the inmate as well as biographical information similar to the intake registers. The forms also include observational notes on the inmate's current behavior, prognosis and possible actions to be taken. Some files give more detail on religious beliefs. Records are restricted.

- The series consists of brief memorandums from physicians to prison authorities usually recommending appropriate medical care or end of need for medical care for incarcerated patients that the physicians have examined. Many memorandums recommend that an incarcerated individual be mustered out of the military squad because of a medical problem. Memorandums include date, addressee, consecutive number, last name of the incarcerated individual, and physicians name and title. The name of physician is often omitted.

- This series consists primarily of microfilmed case records for approximately 3,000 incarcerated males, aged sixteen to twenty years old, processed through the Elmira Reception Center from 1945-1965. The case files represent a ten percent random stratified sample of all individuals processed through the facility and served as source documents for a study in recidivism made by the Department of Correction in 1967. The series also contains documentation of the Reception Center Research Project.

In some instances, remarks such as "sent to hospital" or reduced "to 3rd grade," (loss of privileges and temporary ineligibility for parole) are recorded.
