Personal data includes patient's name; address; occupation; age; sex; color/race; national origin; household composition; and type of residence. Medical data includes diagnosis; probable infection source; food and water sources; sanitary condition; and precautions taken against spread of disease. Records are restricted pursuant to state and federal laws.

The bulk of the records are post cards requesting absentee ballots or notifying the commission of a change in military address. The series also includes envelopes in which official election war ballots were mailed to the commission (or to county boards of election). No actual ballots are present in the series.
