- This series consists of medical and circumstantial evidence collected by State Department of Health epidemiologists to determine the source and type of reported cases of typhoid fever, plus medical information on the course of the illness. Documentation includes a case investigation form detailing vital statistics; history of typhoid fever occurrence and vaccinations; antibiotic treatment; possible sources of exposure to and further spread of typhoid; household and visitor census; and laboratory specimen culturing results. Also included is a narrative case history. Records are restricted pursuant to state and federal laws.

- Among the records in this series are documents from a November 27, 2001 public hearing on the diagnosis and treatment of Lyme disease. That hearing addressed the long-term use of intravenous antibiotic treatment in cases of chronic Lyme disease. Several physicians were under investigation for administering this treatment. Records include written testimony from medical professionals, including supporting medical literature; and correspondence from individuals recounting their experience seeking treatment for symptoms that some physicians linked to chronic Lyme disease.

- The files in this series document administrative proceedings that Department of Health program units or the Bureau of Administrative Hearings are not able to settle without a hearing. Cases involve sanitary code conditions, Medicaid, professional medical conduct, nursing home discharges/transfers, emergency medical services, WIC benefits, and patient abuse. Files include hearing transcripts, motions, intermediate rulings, proposed findings and exceptions, and orders rendered. Series also contains index files that include summary of proceedings, findings of fact, and transcripts for every case that is heard by the judges.
