This series documents instances of inmate misconduct, disciplinary action, and review of the disciplinary action by the institution's Adjustment Committee. Documentation usually includes memorandums and infraction reports describing misconduct; a summary of the infraction and resulting punishment; inmate's explanation; committee decision on the need for further review; and Commissioner of the Department of Correctional Services decision. Files may include inmate letters usually describing situations of misconduct by other inmates.

Summary information on adult inmates is disclosable under law. State Archives staff will review records containing probation, parole, medical, and juvenile criminal history data and disclose information pursuant to the Freedom of Information Law and Personal Privacy Protection Law. The identity of victims of sex crimes is not disclosable. Unless a specific law forbids disclosure indefinitely, all records must be disclosed after 75 years.
This series documents the disciplinary movement of incarcerated individuals into and out of the institution's guardhouse where they were confined in isolation. Entries include consecutive number, surname of the incarcerated individual, date received in guard house, name of receiving officer, date released from guard house, and name of physician for daily visit. In some instances, remarks such as "sent to hospital" or reduced "to 3rd grade," (loss of privileges and temporary ineligibility for parole) are recorded.
