Data includes name, date of admission, age, county and/or municipality of origin, date of commitment, maintenance, and staff remarks. The indexes in this series record the patient's name, casebook volume and page number(s), existence of clinical records, burial record, autopsy record, or prescription record, or date of discharge.

Lists show patients admitted to the facility; date admitted; name; birth date; age; citizenship status; town of residence; legal status; and the name and mailing address of a legally liable relative or guardian. Summary
Prior to 1978, the Department of Mental Hygiene was responsible for services to the mentally disabled and mentally ill, and for treatment oversight. It received patient admission lists from public and private treatment facilities throughout the state. This series consists of admission lists from those institutions under jurisdiction of OMRDD (1978-1983) as well as those Department of Mental Hygiene facilities that were of a type that would have come under OMRDD jurisdiction had it existed during that time period (1927-1978).
