Arrangement
Patient registers (one for women, the other for men): Chronological order by admission.
Indexes: Alphabetical order by patient last name, and reference the casebook pertaining to each patient.
Scope and Content Note
The registers in this series contain handwritten entries of admissions, discharges, and case observations, among other critical patient information. The admission registers list patient admissions and discharge information, recording the individual's name, date of admission, age, county and/or municipality of origin, date of commitment, maintenance, and staff remarks (e.g., date of death, discharge, transfer, escape, etc.).
The indexes in this series record the patient's name, casebook volume and page number(s), existence of clinical records, burial record, autopsy record, or prescription record (each of these indicated with a check mark, if applicable), or date of discharge. The "Record of Male Patients Discharged" contains much of the same information as the "Index" volumes, but also records the patient's age, condition, cause of death, and remarks. The following three fields are penned in: "place of burial," "date of burial," and "county," replacing the formatted fields "duration previous to admission" (i.e., of the patient's condition), "period under treatment," and "form of insanity." Sometimes the date of death or escape is also referred to as "date of discharge" in the remarks field.
