Summary
- This series consists of accident, injury, and escape reports prepared by the ward physician and other staff. The reports usually include date of report, name of patient, patient number, date of admission ward, mental diagnosis, age, date and time of incident. Accident reports also include patient's general mental and physical condition, nature and cause of accident, physician's findings, treatment, witnesses, and persons notified.
