This series consists of continued reports (produced in compliance with Statistics Memorandum No.40, issued by the Dept. of Mental Hygiene, Nov. 1, 1956) regarding patients. Custodial History
This series was transferred to the State Archives from the Middletown Psychiatric Center in June 2000 and accessioned in January 2001.
