Scope and Content Note
This series consists of four small books documenting Willard State Hospital ward admissions, transfers, and censuses. The books were apparently kept for individual units or buildings which are not clearly identified; they do not cover the entire institution.
The volume labeled "Transfers and Admissions" (1929-1971) contains names of patients (all women); date admitted; ward number; date of transfer and ward number; and date of death, parole, or discharge.
"Admission and Transfer Book" (1929-1971) contains name of patients (all women); date; and wards or buildings of admission or transfer. Some entries also include date and cause of death, or date of parole.
"Census Book" (1931-1949) contains patient names (all men); dates admitted, transferred, or other (eloped, returned, died); names of wards or buildings of admission or transfer; current total number of patients in each ward; number of patients home on parole; and total number of patients for all wards.
The volume labeled "Admissions and Transfer Records" (1956-1975) contains date of admission; names (all women); hospital number; religion; ward or building; and total number of patients. The back of this book contains a "Tranquilizer Record" (1955-1967) which includes patient name; identification number; and date tranquilizer use (usually thorazine or reserpine) commenced or ended.

These records were transferred to the State Archives from Willard Psychiatric Center at the time of its closure in April, 1995.
