In addition, the records document treatment of and response to ill consumers and outbreaks of illness. Meeting minutes typically include date of meeting, attendees, brief descriptions of circumstances of several patients' deaths including name, age, living circumstances, medical history prior to death, autopsy results if performed, date and cause of death, and occasionally annotations. Records include minutes for the Syracuse Development Center (1990-2007) and the (Rome) Central New York Developmental Disabilities Services Office (1990-2013).
Access Restrictions
Records identifying patients are restricted permanently in accordance with Section 33.13, Mental Hygiene Law, relating to confidentiality of clinical records. Access permitted under certain conditions upon application to and approval by the Office for People with Developmental Disabilities.
