Scope and Content Note
This series of journals and diaries documents admission and discharge information for specific wards of Manhattan State Hospital and its successor, Manhattan Psychiatric Center. Wards or units are not identified. Entries for individual patients vary and typically include patient name and number, date of admission, and/or date of discharge or transfer to and from ward or unit. If the patient was released to the supervision of another individual, the name and address of the individual is sometimes included. Circumstances of discharge such as patient's improved condition are occasionally given. Some patient deaths are also noted.
Access Restrictions
Restricted in accordance with Mental Hygiene Law, Section 33.13, relating to confidentiality of clinical records. Access is permitted under certain conditions upon approval by the Office of Mental Health.
